New for 2012, our office will also be offering a monthly immunization clinic in Crestline every 3rd Wednesday from 1:00pm - 4:00pm. The Crestline clinic is located at the Calvary United Church of Christ, 511 Heiser Court, Crestine, OH.  View the 2012 Crestline Clinic Schedule.

Vaccine preventable disease rates are at their lowest level ever, however immunization coverage rates remain sub optimal. Educational outreach regarding current vaccines continues to be a priority of the public health nurses. A $10 office visit fee is charged per person per visit. A $10 administration fee is charged per injection for all vaccines. In addition, the cost for vaccines purchased by the health department is also charged to the client. No child is denied required vaccines due to inability to pay.
